Thank You for Choosing Chicago Eagles Water Polo Club

A few important notes for all athletes

1. All Athletes must have a current membership with American Water Polo (AWP) and must renew it every year from initial registration.

2. You will not be ablet to register with CHIWP, without having a current membership

The CHIWP Registration includes:

• Emergency Medical Authorization
• Release Agreement
• Acknowledgement and Consent Clauses
